2024-2025 New York State Health Requirements

All students entering 9th and 11th grade are required by New York State to have an updated Physical Exam on file in the Health Office.

The physical can be dated from 9/1/23 forward to the current date. Please fax to (585)242-7529, mail, email, or bring to the Health Office.

Please schedule an appointment with your child's medical provider as soon as possible.

All students entering 9th grade are required to have one dose of the Meningococcal Vaccine on file in the Health Office prior to the first day of school.

  • If your child had the first dose before 9th grade, then another dose is not required until 12th grade.

All students entering 12th grade are required to have two doses of the Meningococcal Vaccine on file in the Health Office prior to the first day of school.

  • Two doses will be required before 12th grade. Most students entering 12th grade got their first dose when they were younger and are now due for their second dose, or booster. This booster is needed because protection from the vaccine decreases over time.
  • A small number of teens who received two doses before their 16th birthday may need a third dose on or after their 16th birthday to enter 12th grade.
  • The only teens who will not need a second dose before 12th grade are those who got their first dose on or after their 16th birthday.

It's best to check with your medical provider to see whether your child needs any vaccines.

The Meningococcal Vaccine (Menactra, Menveo, Menomune, MenHibrix) is required by New York State to enter school.  Documentation can be faxed to the BHS Health Office at (585)242-7529 or mailed.

The Monroe County Health Department has an Immunization Clinic.  Please call (585)753-5150 for more information.
